02 sensor fault, low vaultage, high vaultage? HELP plz

hi, ive recently replaced my front oxygen sensor, and the rear 1 was replaced not long before, but the engine managment light came today, and when i plugged it in, it came up with 02 sensor, vaultage too high, vaultage too low.? help would be much appreciated, thanks
