3 wire to 2 wire IACV swap problems

Hi all, yeah I know, new member, first post, whatever. I don't own a Honda, but I like them and I've worked on a few of them. That said, I'm not really a Honda car guy (love the bikes though), so I very well might be missing something obvious.

So looooong story short, I helped a buddy of mine swap the intake manifold on his 97 coupe (d16y7) to a side draft intake manifold I ported out to match the 66mm throttle body he has. No idea what the new intake manifold is off of, I suspect an older d-series, it has the separate 2 wire IACV, whereas his obviously has a 3 wire IACV that's part of the TB.
Now here's where things get interesting, I followed the directions on all the guides I've found (which all say the same thing); move pin A14 to empty A12, remove A13, splice on 2 wire connector. I have double and triple checked the wiring, cannot find anything wrong with it. And yet, the idle bounces between 1400-1600ish.
It also throws the P1509 code for the IACV. Now if I clear the codes with the engine off it just throws it again instantaneously. If I clear the codes with the engine running it'll idle up to about 1900ish, then drop right back down to the 1400-1600 surging. And obviously throw the P1509 code. This tells me that there is an electrical issue somewhere, but I cannot find anything wrong with the wiring. We have tried multiple IACV, all know good ones, with the same results.
We also checked for vacuum leaks and sprayed the whole thing down with carb cleaner, can't find any leaks.

Now I'm REALLY hoping someone can come in here and tell me that we did something wrong, cause right now I'm baffled. I am by NO MEANS mechanically handicapped, I'd like to think I have some mechanical skill actually. My buddy who owns the car is learning but not nearly as experienced, hence why I'm asking not him, because he wouldn't be able to explain it in a way you'd all understand.

Other than the idle issue the swap/mod was a success and made a noticeable difference. Now if we can just getting it so my buddy doesn't look like a royal douchbag reving his car in traffic, we'd be golden. XD

Post crystal clear pictures of your ECU wire cutting/repinning and your cut and splice job at the IACV connector. This where you need to focus.
