Advice needed for my 00 vtec b16a

Hello guys i pulled my car down to red line in 4th and my car dropped the valve and made a hole in my 1 piston... What could have cause this... It slagged of as i changed into 5th at abt 215 and then just slagged of but ran for a while when i found the car swithed off... What could have caused this to happen??? I mantained the car well only mods in exhaust and air intake... Help i built a new b16 motor which is ported and gas flowed... And currently running on my car... I dont want to experiance any of the above again... Please help

previous owner did head gasket change on the motor which fudged out...

The best way to keep that from happening is to not drive it like that. Upgraded valvetrain and claying the engine before reassembling to make sure valve and piston arent coming into contact helps too.

Did you have internals in the B16?
