Dying on cold start/idle problem

Thanks to anyone in advance who is taking the time to read this and helping me out.

I have a 97 Honda civc ex model. enginge is a 97 B18b1 with GSR transmission.

Mods are intake and edelbrock throtte body. other than that all stock.

My problem is when start the car in the morning it constantly dies if i dont hold the trottle down for 20-30+ seconds. If i dont hold the throttle it will die instantly. And after the 20-30 sec of holding throttle, i start driving it and when i pull up to stop signs it will die again. its usually at about 200+/- rpm. While im driving there is no hesitation and will rev just fine. When the car reaches operating it will fluctate all the time, no surges or any thing. usaully around 1500rpm. but within the past couple days it's been surging from 1400 to 1500rpm

I've done alot of research and tried everything i read about. since all the problems started within the last couple of months i've tried maybe 3 or 4 iacv. burped cooling system numerous time. took off and cleaned throttle body and replaced with new gasket. checked for vaccum leaks, adjusted idle.

Battery, Alt, and grounds good? Low voltages can cause these problems and have you scratching your head for days. Been there, done that.

ya everythings good battery is year and a half old. alt was replaced when i first got the car because it broke down on me. and all ground are good.
