No washer and it doesn't matter if the wheels turn. If they are actually turning that means that you have it in the grooves on the shaft. Just need to hold it from turning long enough to torque it down correctly. Would want that wheel to come off in your hands while going down the road.

If you are having trouble with the wheel moving while trying to torque it, take the key out and use the security/safety lock as a brace.

If the wheels on the car and the shaft are turning and the steering wheel is not, you don't have it in the grooves in the shaft. Do you have the correct wheel? If it is aftermarket, do you have the correct hub? When the grooves are lined up and it is the correct wheel or hub it slides right on. There is no forcing it, and doing so will ruin the shaft and your only fix would be to replace the column.

I would be very surprised if you can break the lock on the steering wheel when the key is out. It takes a LOT of force to snap a safety lock just by turning the wheel. Trust me.
